Decision Care Services

Introduction

Take a moment (below) to schedule a call with us to discuss the questions you have about our services. This call will take 30 minutes or less and allows us to get to know each other better as we better prepare for a session to best support you.


Care Planning

The majority of our clients are those facing life-changing adjustments following the diagnosis of a parent with a cognitive condition, preventing independent living. While this does not always mean mom or dad must be moved to assisted living, it does require patience, time and creativity to plan for solutions which are safe, thorough and considerate of their needs and the primary family member acting on their behalf.

​As a former in-home senior care-giver, founder, Kristina Twitty, offers a tender hearted conversation with adult children of elderly parents and other family members to walk through questions regarding needs, paperwork, legal steps and recommendations for making adjustments as needed to care for loved ones. Partnering with In-Home Care companies, attorneys and medical professionals, Decision Care Advocates helps bring the experts to you for creating a comprehensive care-plan, one step at at time. 


We will spend up to 90 minutes discussing the reason for your call and needs you have concerning the situation you are facing so we can plan for your next steps. This can be done by video call or in-person for local clients by request. More than one session may be scheduled as needed.

Doctor Visit

No matter what your age or stage of life, doctor visits and medical decisions can be scary and confusing. You do not have to walk alone. Whether you need a good note-taker or an advocate during an appointment, we offer both options.to you or your loved one, so that you are supported by a third party committed to the care of our clients.

(Transportation may be available depending on location for additional charge.)

Family Advocate

Often it is the sudden illness or diagnosis which finds our clients in need of third party support for conversations that can be emotionally charged. With compassion for siblings, parents, spouses and children, we offer facilitation of meetings to communicate needs and wishes for care to your loved ones. This works for family discussion among siblings planning care for a parent as well. Single sessions and packages for short term support are available.


Home Visit - Care Planning

Pastoral Consult

Shepherding a congregation is a ministry to which few are called. Whether you are a Senior Pastor, Women's Ministry Director, Pastor of Seniors, Pastor of Congregational Care or part of your Elder Board, this one is for you. Supporting you with information regarding issues of life and death, technology, grief, or faithfulness in suffering is a cornerstone of our work. Offered as a single issue discussion with resources, a consultation can also serve as a catalyst for further congregational education on a wide variety of issues. Consults are available to as many of your church leadership as you would like to include in one room or on a video conference call.


Medical Advocate

When you need a vocal companion for yourself or a loved one, we can help. As a follow up to Care Planning session, we will work with you to be sure of clear communication and that wishes are understood and honored by your medical team. We will advocate as you direct us, take good notes and provide a summary of the appointment to you and/ or your medical power of attorney as you direct. We ensure that you are supported by a third party committed to your care and also your understanding of options discussed by your physician.
